Signatures may be digitized on a fee for use basis on output from AIS supported printing systems. Depending on the use of the signature, two security levels have been established.

Level 1:

bulletSignatures for forms or output that have no security requirements will be permanently stored in
AIS computing systems for testing and for production printing. The client will sign a Signature Storage Release Form which will be retained as the documentation of authorization.
bulletScheduled productional use of the signature will be made without further client intervention or authorization.

Level 2:

Signatures for forms or output that have security requirements will be handled in the following
manner:

bulletAll secured forms and signatures will be permanently housed on removeable media, such as a diskette. This media will be given to the client and must be returned to AIS for each printing job. No backups will be maintained in AIS for secured forms and signatures.
bulletIn addition to scheduling the job(s) necessary for creation of secured form output, the client will schedule a time for printing of the output. At the scheduled time, the client will present the diskette to AIS Production Services. The output will then be printed while the client remains present for the entire operation. At the conclusion of printing, the operations staff will delete and verify deletion of all secured files.

How to Proceed:

bulletProvide original signature as instructed on the Signature Form.
bulletFor Level 1 forms, complete the Signature Storage Release Form after calling the AIS Help and Support Center at 353-4420, ext. 311.  Help Center personnel will direct your call to someone who can help you with form numbers and any other information you may need to provide.
bulletAttach form(s) to a Request for Application Support and return it to AIS, Room 2 Administration Building.  In the Detailed Description of Request section of the form, refer to the attached forms and provide any additional information necessary.
